Differences in coding systems and payment methodologies contribute to the complexity of facility payment (Table 4). As outlined above, stenting is medically complex, and its practice implications are therefore complicated. The procedure may be performed in the following different places of service (POSs): with the patient either under moderate sedation in an endoscopy unit or operating room or under general anesthesia in the operating room; and with either flexible or rigid bronchoscopy. Necessity of Combined Therapies in Airway Stenting

In managing CAO, combined therapies are routinely required to obtain the best outcomes. For example, endobronchial disease is effectively controlled with Nd-YAG laser ablation, argon plasma coagulation, electrocautery, microdebridement, endobronchial injections/topical applications or coring out, among other technologies.

Airway Stenting

The incidence of central airway obstruction (CAO) is unknown, but an estimated 30% of all lung cancer patients will experience endobronchial disease, which is a major source of CAO. Minimally invasive therapies can be crucial for patients with inoperable disease or limited pulmonary function. Numerous studies have reported that endobronchial therapies, including airway stenting, can palliate symptoms in 80 to 97% of patients with dyspnea, Advanced knowledge and skills are required to insert these stents, and the ability to perform emergency procedures and procedures that require general anesthesia are necessary.

Deliberations about Intensive Care Services in the Veterans Health Administration

This study updates information from the 1990 survey of VHA ICUs and provides the first assessment of regional variation in levels of ICU care in the largest integrated health-care system in the United States. In the two articles reporting on ICU organization in the United States from the early 1990s, results were aggregated to the entire United States. The 1991 survey reported in these studies preceded development of ICU levels, so results were not presented by level of care. Subsequent work using databases from existing sources also preceded development of ICU levels, and use of existing databases limits the ability to assess level of ICU.

Description of VHA ICUs

A total of 213 ICUs were reported at 126 separate locations (Table 3). Seventy hospitals had one ICU, with a range from one to four ICUs. The South had the largest number of ICUs (n = 90). The mix of ICU types varies by region of the country, with single-specialty ICUs dominating in the South, and mixed ICUs dominating in the other three regions. Half the ICUs were single specialty, mostly surgical.

Crown Samples 3.57 Ounces Gold/Ton at Red Lake

Crown Gold Corporation (“Crown” or the “Company”) (TSX VENTURE:CWM) is pleased to announce assays from recent channel sampling of a well-exposed quartz vein on its McKenzie Island property at Red Lake in northwestern Ontario.

Individual channel samples on two segments of the vein assayed up to 122 g/t gold (3.57 ounces/ton) over 0.5 m with most samples (8 out of 14) returning assays greater than 8 g/t gold. Selected channel assay results are tabulated below. Channels CC-05 to CC-16 are sequential along a 25-m segment of vein, while adjacent channels CC-23 and CC-24 are approximately 4 m apart on a vein segment about 45 m to the NNW. Widths are horizontal and approximately 97% of true widths. The segments occur within a 90-m strike length of exposed shearing with quartz veining.
